I assume all risks and hazards incidental to such participation, including risks and hazards incidental to the transportation to and from those activities, and hereby waive, release and absolve the organizers, officers, supervisors, and persons transporting my student whether the result of negligence or for any other cause.

MEDICAL RELEASE
I, parent/guardian of the child named above, at all times at the site of any activity, in case of medical emergency, hereby authorize any hospital, doctor or other licensed medical practitioner, as well as emergency medical treatment personnel, to take what they feel are the correct procedures to aid my child's health and well being. This permission is extended, as necessary to redeemer youth leaders and any local hosting organization of an event or their employees, agents and volunteers. I also hereby assume the responsibility for payment of any such treatment.
ELECTRONICS POLICY - students AND parents must initial that they have read and agree. If this policy is broken, we will take the phone and only return it to a parent or guardian.
RPCYouth trips have a "no electronics" policy, except for a camera. We don't allow any phones on our retreats, not even for camera purposes. Please do not send your child with a phone.
The purpose of this is to remove distraction that is so present in theirs (and our) daily lives as a result of media and devices (all which serve their purpose). We want all our students to spend this time growing in their capacity to enjoy God and know one another through temporarily removing such distractions. So, we are also not allowing video game consoles, iPads, iPods, kindles, headphones, and other things that would fall into the "electronics" category. This is a place where we humbly ask you as parents to trust us by encouraging your children to let go of these things for a few days. As far as phones and contact goes, the leaders will have our phones on us throughout the trip if you need to get in touch with your child in an emergency situation.
If you have questions or would like to discuss this further, please contact either Wes or Morgan before deciding to send your child with a phone anyway. We are humbly and sincerely asking you to trust us with this. We have seen incredible fruit, both socially and individually, from allowing the students to disengage with their devices for the weekend.Thank you!
